Doctor's Office

1. Under the weather - feeling bad, physically (synonym: unwell, sick)
in e.g She was feeling UNDER THE WEATHER because of a flu.

2. Appointment - a meeting in a specific time and place, arrangements to do something or to meet someone (synonym: engagement)
in e.g The class teacher arranged an APPOINTMENT with the school student president.

3. Fill out - to write, complete or type necessary information.
in e.g Please FILL OUT the necessary information at the student's enrollment form.

4. Symptom - a physical indication or sign of an illness or disease (synonym: sign, indication)
in e.g Coughing and a runny nose are SYMPTOMS of flu

5. Diagnosis - the act of knowing the cause or source of the disease or illness
in e.g You need to have a DIAGNOSIS first so we can apply the proper treatment.

6. Infection - being infected or invaded by something
in e.g My eye got an INFECTION so I can't see clearly.

7. Medication - treatment using medicine
in e.g I have to receive MEDICATION so I can recover fast.

Doctor's Office

I was feeling a little under the weather the other day, so I decided to visit my doctor before things got worse. I called her in the morning, and I made an appointment for 9:30 a.m.

When I arrived at her office, I had to fill out some insurance paperwork and a sheet describing my current medical condition, along with the symptoms I was feeling. After a few minutes, the nurse called my name and took me back to one of their examination rooms. She took my vital signs including my temperature and blood pressure, and then I waited until the doctor came in. After looking me over and asking a few questions, the doctor's diagnosis was that I had a bacterial infection. She wrote me out a prescription for some medication, which I was supposed to take two times a day.

 After the exam, I went back to the reception area and paid the bill.
